Found in a BI 34092018 hybrid module, this is either a PMI SW-201 or SW-202 (inverting vs. non-inverting controls): I can't tell from the die image alone.

Files are incorrectly labeled as “SW-06”; found after uploading that subtleties in the datasheet die photos definitively ruled out the SW-06.
